Abstract
After onset of type 1 diabetes, support for integration in kindergarten, day care and school is crucial. This should be performed in close cooperation with the parents. A structured, but also individualised diabetes training for all persons that care for the child should be performed by the local diabetes team. Diabetes-specific knowledge should be addressed to avoid fears and to secure reintegration. This diabetes education is provided in different ways in different regions of Germany. Differences in regulations between federal states and regions or sometimes even within the same town led to heterogeneous models of integration. The working group "inclusion" within the German association for pediatric diabetology has conducted a nationwide survey. 66 pediatric diabetes treatment centres (treating about 9700 children with diabetes) answered the questionnaire. In this paper we present the data on initial diabetes training for persons that care for children at schools, day care or kindergarten after onset of diabetes. Most frequently (90 %) qualified members of a pediatric diabetes team conduct this training usually at the school, kindergarten or day care centre. This initial diabetes education is financed for about 80 % of training for kindergarten or day care centers and about 70 % of schools either with funding, honorary commitment, or is subsidised from hospital sources. That means, that a diabetes training that is essential for the reintegration of children with diabetes and therefore for the positive adherence to a chronic metabolic disease is particularly ensured by personal or private initiative or has to be covered by support of the local clinic with all the problems that occur in consultations with the hospital administration. The working group inclusion therefore calls for a mandatory financial regulation for diabetes training of persons that care for children in school, kindergarten and day care in Germany. This presupposes integration and care of children with diabetes outside of their usual home care.
