Abstract
Background: The therapy of symptomatic uterine fibroids is based on three pillars: surgical therapy, conservative medicinal therapy and interventional therapy with high-intensity focused ultrasound (HIFU) and uterine artery embolization (UAE). In recent years, innovative drugs and techniques have been introduced in all of the three therapeutic categories. Ulipristal acetate (UPA), a selective progesterone receptor modulator (SPRM) is now licensed for the preoperative treatment of fibroids. Surgery and minimally invasive surgery: Surgery is still the most efficient option in the treatment of myoma-related symptoms. In the surgical setting, innovations in laparoscopy and hysteroscopy permit uterus-preserving methods. Minimally invasive surgery reduces perioperative risks, enables outpatient surgery and facilitates a rapid recovery. Therefore, minimally invasive surgery is favored even in terms of health economic aspects. For patients with myoma-associated subfertility, surgery is still recommended as the standard therapeutic option.
