Abstract

Long-term parenteral nutrition (PN) is a therapeutic option for children with short bowel syndrome (SBS), which makes survival possible if life threatening complications are avoided. The composition of parenteral solution takes into account the individual anatomy of remaining bowel and age dependent nutrient requirements. Intestinal failure associated liver disease (IFALD) may lead to the development of liver cirrhosis. Risk factors, treatment options for IFALD and the choice of lipid emulsion including the use of ω-3-containing lipid emulsions are discussed. New concepts for the prevention and treatment of catheter-related complications such as thrombosis and blood stream infections have been developed. The treatment of children with SBS also includes optimized enteral nutrion to stimulate intestinal adaptation, surgical procedures such as intestinal lengthening and intestinal transplantation in patient with irreversible intestinal failure. Intestinal rehabilitation centers which provide highly specialized caretakers have been shown to improve outcome with respect to avoiding PN-associated complications such as IFALD and achieving intestinal autonomy.
