Abstract
The contract about a "Hausarztzentrierte Versorgung (HzV) in Baden-Württemberg" is an innovative health plan in the German system of statutory health care. It is primary-care-driven and based on capitation fee. The results of the implementation stage (2008-10) were evaluated in respect to utilisation and quality of care, particularly the guideline-based care for patients with chronic heart failure (CHF) - both based on routine data, and surveys about general work satisfaction of the practice team (as well as patients) and the employment of practice assistants.
