Abstract
The phase of transition faces chronically ill youth and their families with enormous challenges. This phase bears numerous risks, such as the worsening of health. Under the project named "Growing-up with ModuS", a generic patient education program has been developed that is based on the modular ModuS patient education concept. It comprises a 1.5-day youth workshop, a unit for parents and an interactive web page. Its topics are the transfer to adults' medicine, social legislation, education/profession, and how to get self-reliant while living with a chronic condition. The program was tested with 14 different chronic health disorders under out-patient and in-patient care. The workshops were well accepted by the families and showed positive effects on the self-management. For the future, it is necessary to establish structures that offer similar support to many families in such a special phase of life.
