The HEUREKA project covers the whole of Schleswig-Holstein and aims to provide targeted care for affected overweight children and their families.
Pediatricians and family doctors are the focal point here. They play a key role in identifying the disease and referring their young patients as needed.
HEUREKA ensures, among other things, optimal networking among the stakeholders involved in the care of this patient group, identifying children or adolescents affected by obesity as quickly as possible, and then initiating appropriate care based on the treatment pathway.
Particular attention is paid to the sustainability of the services and the intensive care provided to the young patients – the children and their families should be able to implement and practice what they have learned at home. This is the only way to achieve lasting and healthy weight reduction and thus improve the quality of life for the children and adolescents.
HEUREKA works to integrate the various stakeholders necessary to provide effective and sustainable support. For example, HEUREKA Case Management works to support families in coordinating appointments and thus maintain the ongoing treatment process. Only through continuous support and consistent implementation can therapeutic success be achieved. HEUREKA helps you stay on track!
This project is funded by the Schleswig-Holstein Ministry of Social Affairs, Health, Youth, Family and Senior Citizens. The Institute of Social Medicine and Epidemiology is conducting the data collection and scientific evaluation.
